Practice Closure Update
Thank you for allowing Total Wellness Family Medicine to be a part of your healthcare. As previously announced, the practice is now in the final stages of closure. We are continuing to process medical record requests and previously submitted medication refill requests. Any appointments that were previously accepted will be completed by telemedicine only.
We are no longer accepting routine appointment or medication refill requests. Patients should contact their new primary care provider for ongoing medical care and prescription management.
If you have already submitted a medical records request or medication refill request, or have an appointment that has been accepted, no further action is necessary. We are continuing to process those requests.
Medical records remain available upon request. Please follow the medical records request instructions provided on this website to request a personal copy or to have records transferred to a new healthcare provider.
If you have an urgent medical concern, please contact your current healthcare provider or an urgent care facility. For a medical emergency, call 911.

An automatic email reply will provide instructions for accessing a secure electronic authorization form used for all medical records requests, including secure patient portal delivery, printed paper copies, and unencrypted email delivery.
If you do not receive the automatic response email within a few minutes, please check your spam or junk folder, as some email providers may filter automated messages.
Patients may electronically complete and sign the authorization form through the secure portal without printing or mailing paperwork.
Patients may be asked to create a password or complete a brief verification step in order to securely access and submit the authorization form.
