After many years of caring for patients in our community, Total Wellness Family Medicine will be closing.
Total Wellness Family medicine will permanently close its physical office location effective June 10, 2026

Patients should not delay establishing care with a new healthcare provider to avoid interruptions in medical care, prescription management, or follow-up testing.
Following the June 10, 2026 office closure, limited telehealth appointments may remain available for up to 30 days to assist with continuity of care, medication management, follow-up testing, and transition planning when medically appropriate.
Patients may wish to consider the following resources when locating a new healthcare provider:
Patients with urgent or immediate medical concerns should seek care through an urgent care facility, emergency room, or by calling 911 when appropriate.
For continuity of care, the preferred method of medical records transfer is for your new physician or healthcare provider to initiate the request directly from their office after you establish care with them. In most cases, your new provider's office will supply you with a medical records authorization form for you to review and sign. Your new provider may then fax the completed authorization request directly to our Medical Records Fax Number: 702 965-2603.
Email requests may be submitted to twfmrecords@hushmail.com.
An automatic email reply will provide instructions for accessing a secure electronic authorization form used for all medical records requests, including secure patient portal delivery, printed paper copies, and unencrypted email delivery.
Patients may electronically complete and sign the authorization form through the secure portal without printing or mailing paperwork.
Patients may be asked to create a password or complete a brief verification step in order to securely access and submit the authorization form.
For faster processing, patients may include one of the following delivery preferences in the subject line of their email request:
Please allow reasonable processing time for medical records requests during the office closure transition.
Patients selecting Secure Patient Portal Delivery will receive instructions for securely viewing and downloading their medical records through the online records request portal once records are available.
No special application or subscription service is required to access the secure portal.
Printed paper copies requested directly by the patients may be mailed to the address provided on the authorization request form. Copying fees of $0.60 per page plus postage may apply consistent with Nevada regulations and applicable law.
Patients may request records through unencrypted email; however, patients should understand that standard email transmission may carry privacy and security risks, including possible unauthorized access or interception of medical information.
Some email providers may limit attachment sizes, which could affect the ability to electronically transmit larger medical records files through standard unencrypted email delivery. In such cases, alternative delivery methods may be better suited.
Total Wellness Family Medicine will make reasonable efforts to assist patients requiring special accommodations related to disabilities, accessibility concerns, or communication needs. Patients needing assistance or alternative arrangements for obtaining records are encouraged to contact the office at (725) 316-5230 or emailing twfmrecords@hushmail.com so reasonable accommodations may be discussed when possible.
