Total Wellness Family Medicine has permanently closed and is no longer providing patient care.
Requests for prescription refills, prior authorizations, new orders, referrals, or other ongoing clinical care should be directed to your current healthcare provider.
IMPORTANT - CONTINUITY OF CARE
Patients who had laboratory testing, imaging/radiology studies, referrals, consultations, or other diagnostic testing or treatment initiated or ordered through Total Wellness Family Medicine should discuss these with their current healthcare provider. This includes anything that is pending, incomplete, or completed but may require additional review or follow-up.
Please ensure that your current healthcare provider has access to the relevant records and results so they can determine whether any additional evaluation, testing, treatment, or follow-up is needed.
Medical record requests and correspondence concerning care previously provided by Total Wellness Family Medicine may continue to be submitted through the designated medical records process.
Medical records remain available upon request. Please follow the medical records request instructions provided on this website to request a personal copy or to have records transferred to a new healthcare provider.
If you have an urgent medical concern, please contact your current healthcare provider or an urgent care facility. For a medical emergency, call 911.

An automatic email reply will provide instructions for accessing a secure electronic authorization form used for all medical records requests, including secure patient portal delivery, printed paper copies, and unencrypted email delivery.
If you do not receive the automatic response email within a few minutes, please check your spam or junk folder, as some email providers may filter automated messages.
Patients may electronically complete and sign the authorization form through the secure portal without printing or mailing paperwork.
Patients may be asked to create a password or complete a brief verification step in order to securely access and submit the authorization form.
Please allow reasonable time for processing your request.
Please submit a written medical records request accompanied by a patient-signed authorization for release of medical records.
Please include the patient's full name and date of birth, the records being requested, and the name, address and telephone number and fax number of the receiving healthcare provider or practice.
Please fax the written request and patient-signed authorization to the Total Wellness Family Medicine Medical Records Fax at (702) 603-3115.
An automatic practice-closure response may be generated upon receipt of your fax. Medical Records requests continue to be accepted at this fax number, and a complete request does not need to be resubmitted.
