Total Wellness Family Medicine

Dr. Kimberly Adams-Cortez is providing limited continuity-of-care appointments through August 14, 2026, for existing patients with active medical concerns who have not yet established care with a new healthcare provider. Appointments are based on medical appropriateness and scheduling availability.

After August 14, 2026, Total Wellness Family Medicine will no longer be scheduling patient appointments. Patients who have not already established care with a new provider are strongly encouraged to do so promptly to avoid interruptions in medical care.

Appointment details, including the location and arrival instructions for approved in-person visits, will be provided when an appointment is confirmed.

Appointment requests may be submitted through August 14, 2026.

Please note: Appointment availability is limited and intended to assist existing patients with active medical concerns while they establish care with a new healthcare provider. Ongoing primary care will not be available after August 14, 2026.

An automatic email reply will provide instructions for accessing a secure electronic authorization form used for all medical records requests, including secure patient portal delivery, printed paper copies, and unencrypted email delivery.

If you do not receive the automatic response email within a few minutes, please check your spam or junk folder, as some email providers may filter automated messages.

Patients may electronically complete and sign the authorization form through the secure portal without printing or mailing paperwork.

Patients may be asked to create a password or complete a brief verification step in order to securely access and submit the authorization form.
